The goal of the Creston Middle School Athletic Department is to provide the best opportunity for their athletes to excel in teamwork, sportsmanship, self-discipline, and moral character. The purpose is to provide participants with experiences that will be positive, memorable, and those that will help them to develop the capacity for commitment to a cause, acceptance of responsibility, and loyalty toward any chosen endeavor. It is hoped that these traits will be carried with the athletes to the Warren Central High School Athletic Program.

The Creston Middle School Athletic Handbook is provided to each student-athlete before the start of a sports season. The handbook includes the policies and rules of the athletic department. It is also provides general information such as directions to opposing schools and ticket information. Athletic Handbooks are available at any time in the athletic office.
Every student must have a current physical on file before they can try out and/or participate in any sport. Physical examination is valid for the next school year if a physician signs it on or after April 1st. This policy will not be waived as it is an IHSAA rule. Please click on the link to download a copy of the required <<physical form>>.
In order to participate in athletics at both the middle school and high school levels in Warren Township, parents must read, sign, and return to the Athletic Office of their school the Concussion Acknowledgement and Signature Form for the 2013-14 school year and beyond. Student-athletes will not be allowed to participate (this includes conditioning and practice) in any sport until this form is on file with their school’s athletic office. Please click on the link to download a copy of the required <<Concussion Management Form>>.
Creston Middle School administrators and coaches are safety conscious and are trained to instruct athletes in the safe and proper techniques of their individual sports. Due to the nature of athletic activity injuries may occur. While the school provides the opportunity for such participation, the parent retains the right of denial of such participation and must carry the responsibility for providing medical care and insurance coverage for their son or daughter. Parents are required to have an insurance policy to cover athletic injuries and the cost of treatment. Contact the Athletic Department for information about insurance plans, which can be purchased by parents through the school to supplement your family medical policy.
IT SHOULD BE NOTED THAT NEITHER CRESTON MIDDLE SCHOOL NOR WARREN TOWNSHIP SCHOOLS CARRY ANY KIND OF FIRST DOLLAR MEDICAL INSURANCE FOR ATHLETIC INJURIES.
